Referral Information at a Glance

Including relevant supporting information with your referral is always appreciated. With your patient’s consent, please consider attaching:

• Mental Health Care Plans
   
• Clinical Psychology reports or other Allied Health input
   
• Any other recent or relevant clinical documentation
   

A baseline set of observations—including heart rate and blood pressure—is also very helpful and will prevent your patient from needing to arrange these separately.

Please note that you do not need to order any specific investigations prior to referral; I will organise any new tests or assessments as clinically indicated.

We are only able to accept referrals for patients aged 21 years and over.
